When it Comes to Knowing Your Family and Child, However,
We Do Like to Take Some Time.
The Parent Interview and Child Assessment
Gives Us Valuable Insight Into Your Goals
and Your Child's Needs.

Bring your child to this meeting with the BCBA. During this time the BCBA will help determine if we can meet your child's needs answer your questions and determine if our clinic is a good match for you and your family.  We will gather information about your child, observe and interact with your child.  Any recent assessments or evaluations will be helpful and need to be sent to the main office prior to your visit or brought with you to the meeting.

  1. Children must be at least 10 months of age.
  2. Applications are processed and selection is made according to the staff availability and the child’s profile in relation the level of services are needed to provide the appropriate intervention.  No child is guaranteed enrollment. 
  3. The Shape of Behavior reserves the right to request a full and complete physical as a condition to enrollment.  The child must not have a communicable disease.
  4. The Shape of Behavior requires a $1,000.00 deposit to hold your child’s place for services.  This deposit is returnable when you give 30 days notice to withdraw from treatment or it can also be applied to your child’s last month of services.  This deposit is also used if the start date is changed by the parent/guardian.
